Forms and optional tub photos are processed by FormSubmit and delivered to our business email. FormSubmit states that it retains text submissions for 30 days and does not retain uploaded files in its submission archive. We keep received emails and attachments as needed to respond to your request and manage your project. We do not sell form submissions. Please send only photos of the tub and surrounding space and omit medical information.

Referrals

Care professionals, family members, or organizations may refer someone to us through our referral form. A referral may include the client's name, phone number, ZIP Code, and the best time to reach them, along with the referrer's name, organization, and contact details.
